Care Manager: Proactive Population Health Navigator

Family Health Services is seeking a full-time Care Manager to work within the medical home team in Idaho. You will drive proactive care using registries and evidence-based guidelines, coordinate services, address barriers, and monitor health outcomes for a provider panel.

You will facilitate communication, track care plans, and collaborate with internal and external resources to ensure patient-centered care and successful health outcomes.
