Supporting the Transitional Care Coordinator and Inpatient Social Worker, the full-time Care Management Resource Center Specialist will manage post-acute pre-certification processes, facilitate communication between care management staff and agencies, and document discharge transition plans while working remotely after initial training and attending on-site meetings once per month. Key responsibilities Initiate contact with payers for precertification and follow up on transfer to post-acute facilities Handle referrals and facilitate placement with post-acute agencies while managing discharge plan communications Complete data tracking and collection to identify trends and assist with routine statistics Required qualifications High School Diploma or GED Three to five years of administrative/clerical experience in healthcare or a related field Fluency in medical terminology Computer literacy Excellent organizational skills
