We are currently seeking Care Coordinator to join our OhioRISE program. As a Care Coordinator, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports.
You'll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community.Key ResponsibilitiesAdheres to the CareStar Rule in performance of job responsibilities.Understands and complies with CareStar Policies and Procedures.Maintains confidentiality as related to patient information.Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.Conducts face-to-face eligibility evaluations, including but not limited to, administration of Needs and Strengths Assessments; verification that the individual resides in a program approved environment; other eligibility criteria as described in program specific guidelines ensuring that such evaluations are completed in accordance with legal, program, contract and/or CareStar requirements. Develops, monitors, and updates individual's person-centered care plans to ensure the individual's needs, preferences, health, and welfare.Assists individuals with connections to needed resources, including services, medical, social, educational, and other resources, regardless of funding source.Identifies, reports, and addresses all incidents as defined in the program specific guidelines, following the process outlined by program specific guidelines.Documents all care coordination activities in a record to meet all requirements, including accuracy, timeliness, and content requirements using the process prescribed by the program specific guidelines.Coordinates and authorizes all program services.Provides services in a manner that supports and respects the individual's communication needs, including translation services, and/or assistance with communication devices.Identifies when an individual experiences a significant change (as defined by the process as stated in the program guidelines), and conducts an evaluation to determine if a change to the type, amount, or scope of services is warranted.Minimum QualificationsHigh School Diploma - 3 years of experience required.Bachelors/Associates Degree - 2 years of experience required.Masters Degree or higher - 1 year of experience required.Must have experience working with youth, and their families, who meet a functional need for behavioral healthcare.Have knowledge of issues affecting individuals served and community-based interventions/resources for those individuals (e.g., Social Determinants of Health, etc.).Have effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds.Have the ability to utilize critical thinking and independent judgment, along with multiple points of data/input to identify issues, develop solutions, and exercise judgment to meet requirements.Must be available to work within CareStar's business hours of 8am-5pm.Are familiar with suite of Microsoft Office programs.Possesses a valid driver's license and car insurance as required by State law.Why Join CareStar?Remote flexibility with meaningful community engagementCompetitive salary based on experience and educationComprehensive benefits: Medical, dental, vision, life insurance401(k) with a generous company matchPaid time off + 10 paid holidaysEmployee Stock Ownership Plan (ESOP) – become a part-owner in the companySupportive, mission-driven culture focused on improving lives
