To support youth with complex needs and their families, the full-time Care Coordinator - Ohio Licensed will engage with clients, conduct assessments, and develop care plans while working remotely from Wood County. Key responsibilities Effectively engages children/youth with significant behavioral health needs and their family/caregivers for an assigned caseload Conducts comprehensive CANS assessments and facilitates the development of individualized Child and Family-Centered Care Plans Facilitates Child and Family Team meetings to ensure the child and family have a voice in their care plan and access to necessary resources Required qualifications At least 3 years of experience in community-based services for children and youth, or relevant education with reduced experience Background in family systems, community resources, case management, or related fields Culturally competent with experience managing complex cases involving children and families Ability to complete high-fidelity wraparound training within specified timeframes Valid Driver's license and acceptable driving record, with the ability to meet internet service requirements for remote work
