To support a growing pediatric practice, the part-time Care Coordinator - New Jersey will provide clinical and clerical support, manage patient communications, and ensure quality care in a remote environment. Key responsibilities Manage incoming and outgoing patient communications, including clinical inquiries and lab result notifications Ensure pre-visit preparations are completed and follow up with patients regarding appointments and care transitions Assist the care team in completing necessary forms and reports to manage patient health needs effectively Required qualifications High-school level education or equivalent 2-4 years of healthcare experience in outpatient clinics, preferably in specialty/multispecialty settings Spanish fluency preferred
