Care Coordinator, MA (Bilingual) (Fully Remote)

Your Role: Support proactive care coordination for patients with complex chronic conditions including CKD. Conduct high-volume text and phone outreach to engage patients and reinforce care plans. Maintain audit-ready documentation and ensure HIPAA compliance. What You'll Do: Enroll and onboard patients into CCM programs, verifying eligibility and consent. Coordinate referrals, care transitions, and follow-up appointments with providers and partners. Use EHR and CCM platforms daily to manage patient messaging and collaboration.
