To ensure cost-effective delivery of quality care, the full-time Care Coordinator IV will facilitate a team approach with the Interdisciplinary Care Plan team, conducting health risk assessments and coordinating care for clients with chronic or complex conditions. Key responsibilities Facilitates collaboration with the Interdisciplinary Care Plan team to address care issues and specific member needs Conducts in-depth health risk and comprehensive needs assessments across various parameters Implements and monitors care coordination strategies for members requiring intensive interventions Required qualifications Master's degree with 4 years of experience, or Bachelor's degree with 8 years of experience, or Associate's degree with 9 years of experience, or 12 years of relevant experience in lieu of education Proficiency in Microsoft Word, Excel, and Outlook Experience analyzing trends using decision support systems Business management skills, including cost/benefit analysis and negotiation Knowledge of referral coordination to community and private/public resources
