Providing administrative and coordination support to the interdisciplinary Care Team, the full-time Care Coordinator DSNP will manage member outreach, conduct assessments, and facilitate access to services while working remotely. Key responsibilities Welcome and orient new members to the DSNP program, conducting outreach and assessments Support the development and implementation of each member's Individualized Care Plan (ICP) and coordinate access to necessary services and resources Educate members and caregivers on plan benefits and follow up to address barriers to care Required qualifications 2+ years of experience in care coordination, customer service, or healthcare Experience in a managed care or HMO setting is strongly preferred Understanding of the needs of older adults and disadvantaged populations Bilingual (Spanish) is required Bachelor's degree in social or health sciences is preferred
