Join us in making a real difference in the health of our community.Job SummaryPosition title: Care CoordinatorJob Summary: The Care Coordinator is responsible for building and maintaining strong relationships with patients, conducting in-person assessments, and developing individualized care plans in coordination with healthcare providers and caregivers.
