Join us in making a real difference in the health of our community.Job SummaryPosition title: Care CoordinatorJob Summary: The Care Coordinator is responsible for building and maintaining strong relationships with patients, conducting in-person assessments, and developing individualized care plans in coordination with healthcare providers and caregivers. The ideal candidate will serve as a trusted advocate and central point of contact for patients, helping them navigate the healthcare system and access the resources they need to support their health and well-being.ResponsibilitiesBuild and maintain trusted relationships with patients through ongoing communication and supportConduct face-to-face assessments to evaluate patients' medical, social, and emotional needsDevelop personalized care plans in collaboration with providers, caregivers, and the care teamCoordinate and connect patients with appropriate community resources to address social determinants of healthPromote timely access to care, including preventive services and follow-up appointmentsFacilitate referrals to primary care and specialty providers as neededAssist with scheduling appointments and arranging transportation servicesServe as the main point of contact and advocate for patients, caregivers, and the broader care teamEducate patients on available health services and community support programsMaintain accurate and up-to-date documentation in accordance with organizational and regulatory standardsEnsure high-quality, patient-centered care that improves outcomes and patient satisfactionQualificationsBachelor's degree in healthcare, social work, public health, or a related field preferredAssociate's degree accepted with relevant experience in healthcare or social servicesMinimum 2 years of experience in a healthcare settingPrior experience in case management or care coordination is preferredStrong interpersonal and communication skillsAbility to work independently and collaboratively within a care teamProficient in documentation and electronic health records (EHR) systemsKnowledge of community health resources and patient advocacyWhat We Offer:Competitive hourly rate: $22–$25/hrSupportive team environmentOpportunities for professional developmentThe chance to make a meaningful impact on patient health outcomesEqual Opportunity EmployerEssen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.
