To support a growing telehealth program, the full-time Care Coordination Specialist will manage patient engagement, triage, and case management while working remotely with patients in Pennsylvania, New Jersey, New York, and Delaware. Key responsibilities Serve as the first point of contact for patients, assessing urgency and triaging requests to the appropriate team Manage an active caseload by developing individualized care plans and maintaining consistent communication with patients Document patient interactions and collaborate with the clinical team to ensure high-quality care delivery Required qualifications 2+ years of experience in care navigation, case management, or patient-facing customer support in a healthcare setting Working knowledge of community resources and systems in PA, NJ, NY, and/or DE Proficiency in using EMR and care management platforms High school diploma or GED required; associate's or bachelor's in social work, public health, or human services strongly preferred Ability to manage a caseload and prioritize tasks across multiple communication channels
