Care Coordination Product Leader Greenfield Vision

Veradigm is seeking a Director of Product Management for Care Coordination to own and shape the care coordination and chronic disease management product portfolio across provider and payer segments. You will set strategy, prioritize features, and drive discovery through delivery, with greenfield responsibilities and strong executive collaboration.

You will start as an individual contributor, then build and mentor a PM team, guiding cross-functional work with Engineering, Design, Sales,
