Care Coordination Case Manager

Owning the continuity of care for patients needing higher levels of support, the full-time remote Care Coordination Case Manager will identify, assess, and connect patients to appropriate care options while collaborating with clinical teams to ensure seamless transitions. Key responsibilities Identify patients requiring more intensive care than the virtual outpatient model can provide Assess patient needs and barriers to care through phone or video consultations Plan and execute individualized transition-of-care plans, including scheduling appointments and facilitating referrals Required qualifications Bachelor's degree in social work, nursing, psychology, public health, or a related field; master's degree preferred 2+ years of experience in case management, care coordination, or a similar role in a behavioral health or healthcare setting Working knowledge of behavioral health levels of care and experience with older adults, Medicare, or Medicare Advantage Clinical licensure or certification (e.g., LMSW, LCSW, RN) is a plus Comfortable with telehealth platforms and able to learn new tools quickly
