Care Coordination Case Manager (Fully Remote)

• Own continuity of care for patients needing more than virtual outpatient therapy.
• Work remotely from anywhere in the US as a full-time W2 team member.

Key Responsibilities:

• Identify high-acuity patients and assess their needs, preferences, and coverage.
• Build individualized transition-of-care plans and connect patients to appropriate services.
• Execute referrals, schedule appointments, transmit records, and confirm attendance.

• Calm, relentless problem-solvers with 2+ years of healthcare case management experience.
• Strong communication skills and comfort with telehealth platforms and EMRs.
