To support families navigating the complexities of aging, the part-time Care Consultant will serve as the first point of contact for prospective clients, providing empathetic guidance and support while managing outreach and engagement efforts remotely. Key responsibilities Serve as the primary contact for prospective clients, responding to inquiries and guiding families through available services Assess caregiver and patient needs to determine appropriate care pathways and manage outreach efforts to meet performance goals Collaborate with clinical and service teams to ensure seamless communication and support for families requiring additional resources Required qualifications 2+ years of experience in customer service, client success, or care coordination roles Experience working with older adults or family caregivers in healthcare or social services environments Familiarity with dementia care or aging-related conditions is preferred Comfortable using technology and operational tools such as CRMs and EHRs Demonstrated ability to work towards performance goals in a fast-paced environment
