You will apply your knowledge and skills in the care of patients with end-stage disease, playing a key role in their holistic evaluation. Your strong critical thinking, interpersonal communication, conflict management, problem-solving, autonomy, and decision-making skills will be essential.To be successful in this role you will demonstrateProfound knowledge in the care of patients with end-stage cardiovascular disease.Exceptional critical thinking and problem-solving abilities.Superior interpersonal communication and conflict management skills.Strong autonomy and decision-making capabilities.ResponsibilitiesCoordinates the referral process and introduction of patients for purposes of evaluation and assessment.Receives incoming referrals from physicians & hospital & responds quickly to facilitate rapid access to the system.
