To support the mission of advancing health equity, the full-time California Patient Care Coordinator will conduct high-volume telephonic outreach to prospective CalAIM members, educate them on available services, and facilitate their enrollment into Enhanced Care Management and Community Supports programs while working remotely. Key Responsibilities Conduct high-volume outbound telephone outreach to engage prospective CalAIM members and explain available services Screen potential participants for program eligibility and schedule enrollment appointments with Care Managers Document outreach activities and maintain productivity standards related to call volume and enrollment goals Required Qualifications High school diploma or equivalent Minimum of one (1) year of experience in patient-centered care, healthcare outreach, or related fields Proficiency with Microsoft Office and web-based applications Ability to manage a high volume of outbound calls while maintaining professionalism and empathy Experience working with Medicaid/Medi-Cal populations is preferred
