Key Responsibilities Provide care coordination and clinical oversight for assigned patient caseloads across both agencies Review, audit, and sign clinical documentation, including Starts of Care (SOC) and Discharges Serve as the point of contact for incoming clinical inquiries from field staff and patients Required Qualifications Active, unrestricted California Registered Nurse (RN) license Minimum of 1-2 years of hands-on home health RN experience Minimum of 1 year of supervisory experience in home healthcare or a similar field Strong proficiency with Kinnser/WellSky EMR strongly preferred Ability to fulfill the duties of this role remotely with strong digital communication skills
