California Licensed RN Care Navigator

To support kidney health management, the full-time California Licensed RN Care Navigator will manage a caseload of patients, facilitating care coordination and delivering education on renal health while working remotely within the PST and MST time zones. Key responsibilities Handle inbound and outbound calls to provide exceptional service and support to members Educate patients on kidney health, dialysis options, and related co-morbid conditions to optimize care Collaborate with healthcare teams to coordinate care and resolve patient concerns effectively Required qualifications Active, unrestricted RN license required Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree CCM certification preferred Three (3) years of experience in case management preferred Experience in a dialysis or transplant center preferred
