To elevate patient care at a nationally recognized Academic Medical Center, the part-time California Licensed Oncology Pharmacy Specialist will provide comprehensive medication management, collaborate with care teams under a Collaborative Practice Agreement, and conduct patient assessments both virtually and in-person. Key responsibilities Provide comprehensive medication management, including reconciliation and dose optimization Partner with interdisciplinary teams to order individualized care plans and promote medication coordination Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as appropriate Required qualifications Doctor of Pharmacy (PharmD) degree Active pharmacist license in California Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with at least 3 years of clinical oncology pharmacist experience At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement (CPA) Strong computer skills, including proficiency in Microsoft Office tools
