Delivering exceptional remote care, the full-time California Licensed Nurse Practitioner will manage comprehensive, patient-centered care for adults with complex medical, emotional, and social needs using telemedicine tools. Key responsibilities Develop and implement individualized care plans in collaboration with patients and interdisciplinary team members Provide clinical care, including assessment, treatment, and monitoring of acute and chronic conditions Address behavioral health concerns and social determinants of health by connecting patients with community resources Required qualifications Active and unrestricted licensure as a Nurse Practitioner in California Minimum of 3 years of clinical experience, preferably in primary care with a focus on complex care Proven ability to manage a patient panel and provide longitudinal care Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics preferred Proficiency in telemedicine technology, including EHR systems and videoconferencing platforms
