California Licensed Nurse Care Manager

To support a growing healthcare practice, the remote California Licensed Nurse Care Manager will review electronic health records to extract relevant clinical and behavioral information, create compliant care plans for patients in a Dual Eligible Special Needs Plan, and maintain productivity standards by completing a minimum of 100 care plans per week. Key responsibilities: Review electronic health records to gather pertinent information for care plan development Create accurate, complete, and CMS-compliant care plans based on documentation analysis Collaborate with managers and quality reviewers to resolve documentation questions and ensure timely care plan completion Required qualifications: Active unrestricted RN license in the state of California Minimum of 3 years of clinical experience in care management or related fields Strong understanding of CMS compliance requirements and care planning processes Proficiency in electronic health records and care management platforms Experience with Medicare Advantage or CMS-regulated documentation is preferred
