Seeking a California Licensed Nephrology NP/PA, the fully remote position will evaluate, diagnose, and manage chronic kidney diseases while conducting follow-up visits and collaborating with care teams in a telehealth environment. Key responsibilities Evaluate, diagnose, and manage chronic kidney diseases Conduct follow-up visits and provide longitudinal care for established patients Document patient encounters accurately in the EMR and collaborate with specialists for coordinated care Required qualifications CA Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in good standing Minimum 2 years of Nephrology experience in outpatient or telehealth settings Comfortable practicing autonomously in a remote care environment Reliable internet connection and familiarity with telemedicine technology Strong clinical documentation skills
