California Licensed Clinical Pharmacist

To support a growing healthcare technology company, the part-time California Licensed Clinical Pharmacist will engage with patients and providers over the phone to optimize medication therapy, conduct medication reviews, and enhance patient care using Arine's software platform. Key responsibilities Conduct Medication Therapy Management services, including Comprehensive and Targeted Medication Reviews, and provide adherence counseling Evaluate and document patient data in the software platform, preparing reports and following up with clinics and pharmacies to resolve issues Collaborate with the Clinical Support team and Engineering to improve workflows and enhance the software platform Required qualifications Doctor of Pharmacy (PharmD) from an accredited college/university Licensed Pharmacist in good standing, with multiple state licenses being a plus Knowledge of chronic disease states and medication management, including CMS quality measures Experience engaging with diverse patient populations and promoting medication adherence Strong computer skills, including proficiency in Google Workspace and comfort using various collaboration tools
