Connecting with prospective clients, the full-time California Licensed Clinical Outreach Specialist will engage in client outreach, facilitate enrollment in Enhanced Care Management services, and collaborate with care teams, all while working remotely. Key responsibilities Engage with prospective clients through 60-100 outbound calls daily and record interactions in the Client Relationship Management (CRM) system Assist clients in completing psychosocial assessments and developing personalized care plans Review medical charts to document client needs and recommend appropriate interventions Required qualifications Active California Licensed Vocational Nurse (LVN) license, Medical Assistant certificate, or Bachelor's degree in a related field Minimum of 2 years of telephonic outreach or enrollment experience in a high call volume environment Experience working with complex care clients and ECM Populations of Focus Comfortable with technology, including video conferencing and electronic health records Bilingual (English/Spanish) highly preferred
