To support a growing Enhanced Care Management team, the full-time California Licensed Clinical Intake Specialist will conduct client outreach, perform intake assessments, and connect clients with appropriate resources while working remotely. Key responsibilities Conduct outreach to newly referred clients and log activity in Salesforce Perform 8-12 intake visits daily, ensuring efficient assessments and care plan development Communicate critical client information to the care team for personalized care planning Required qualifications Active, unrestricted California Licensed Vocational Nurse (LVN) license, Bachelor's degree in a related field, or Medical Assistant certificate Minimum of 2 years of intake or admissions experience in a high call volume environment Experience working with complex care clients and ECM Populations of Focus Proficiency in Health Gorilla or Health Record Systems Bilingual (English/Spanish) highly preferred
