Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop case management plans while working remotely. Key responsibilities: Evaluate patient needs and goals to determine eligibility and develop client-focused case management plans Coordinate resources and services to ensure continuity and quality of care, including monitoring patient progress toward treatment milestones Assist in the development and communication of comprehensive disposition plans in collaboration with patients, caregivers, and healthcare providers Required qualifications: Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field Experience in coordinating patient care and managing a caseload of low-risk patients Knowledge of regulatory, accreditation, and regional guidelines related to case management Ability to document and maintain accurate patient records in compliance with established standards Familiarity with community resources and referral processes for patient services
