To support high-risk Medicare Advantage members, the full-time California Licensed Bilingual RN will manage and coordinate care for medically complex patients in a fully remote environment while collaborating with multidisciplinary teams to enhance patient outcomes. Key responsibilities Manage and coordinate care for medically complex Medicare Advantage members Build trusted relationships with members and caregivers through ongoing virtual engagement Lead transitions of care following hospital and inpatient discharges Required qualifications Located in the South Bay - San Diego, CA area Bilingual in Spanish and English Minimum 3 years of RN experience in complex care, care management, or related clinical settings Experience managing medically complex, high-risk patient populations Active, unrestricted RN license in CA and current BLS certification
