To enhance the ability of frail seniors to age in place, the part-time California Licensed Assessment RN will conduct telephonic assessments, develop care plans, and provide health education while working remotely during Pacific Time business hours. Key responsibilities Perform initial and annual telephonic assessments for dually eligible members and determine Nursing Facility Level of Care Evaluate assessment findings to ensure clinical appropriateness of member-centered care plans and develop comprehensive care strategies Provide education and coaching for chronic conditions to promote member engagement and successful health outcomes Required qualifications Associate's degree in nursing (ADN/ASN) required; Bachelor's degree in nursing (BSN) strongly preferred CA Registered Nurse (RN) license required 3+ years of experience in managed care, healthcare environment, or case management Bilingual in English/Spanish preferred 2+ years of experience working with seniors and remotely strongly preferred
