CACP Outreach Specialist

To support the Comprehensive Advance Care Planning (CACP) program, the full-time CACP Outreach Specialist will conduct high-volume patient outreach through inbound and outbound calls, educate patients about advance care planning, and manage appointment scheduling while working remotely across various U.S. time zones. Key responsibilities Conduct high-volume outbound calls to educate patients on Advance Care Planning and schedule appointments with Facilitators Respond to inbound patient inquiries, manage referrals, and maintain digital patient communications through multichannel systems Adhere to approved scripts and workflows while achieving key performance metrics and maintaining HIPAA compliance Required qualifications High School Diploma At least 1 year of telephonic outreach, customer service, or tele-sales experience in a call center environment Ability to navigate telephony platforms, web applications, and CRM software Strong active listening and verbal communication skills with an empathetic approach Access to a reliable high-speed internet connection
