Bilingual Social Worker, PACE Care & Counseling

CalOptima Health invites applications for a Social Worker (PACE) role, bilingual required, to join the PACE Clinic team and provide direct social work care management services. The position supports participant screening, case management, counseling and referrals, with travel in Orange County as needed.

Responsibilities include IDT participation, care planning, safety assessments, and coordination with community partners.
