Bilingual Social Care Coordinator

To support New York's Medicaid Section 1115 Waiver program, the part-time Bilingual Social Care Coordinator will engage Medicaid members, assess eligibility for Health-Related Social Needs services, and coordinate referrals through Epicured and community partners in a remote setting. Key Responsibilities Manage inbound and outbound member communications via phone and email, utilizing various partner technology platforms Conduct outreach to screen potential members, assess eligibility, and guide them to service providers based on identified health-related social needs Track and follow up on referrals to ensure service delivery and member satisfaction Required Qualifications High school diploma (Associate degree preferred) 3-5 years of experience in customer service, call centers, social services, or care navigation Experience in a virtual contact center environment (Five9 preferred) Fluency in Mandarin or Cantonese and English Ability to manage high-volume interactions in a compliance-driven setting
