This role is responsible for delivering comprehensive case management services across the continuum of care, assessing, planning, implementing, coordinating, and evaluating care for assigned consumers.
The ideal candidate must hold an RN licensure in Iowa, be bilingual in Spanish, and have a minimum of 2 years clinical practice, preferably with case management experience. The position offers the option to work remotely as well.
