Bilingual Remote LPN

Supporting patients enrolled in chronic care management and remote patient monitoring programs, the full-time Bilingual Remote LPN (Care Navigator) will provide education, conduct phone consultations, and maintain patient records while working remotely in a patient-centered environment. Key Responsibilities: Complete phone consultations with patients to provide support and education about their chronic conditions Educate patients on health management strategies and assist them in achieving their provider-approved care goals Maintain accurate patient records and document all interactions and care plans per protocol Required Qualifications: Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN) 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management Ability to communicate fluently in Spanish, both verbally and in writing Proficiency in using electronic health records (EHR) and care management software Reliable internet connection and proficiency with virtual communication tools
