Heartline Staffing is recruiting a Primary Care Physician to provide value-based healthcare to Medicare patient populations. Physicians are supported by large care teams so they can focus on quality of care rather than volume of services. The value-based care model focuses on outcomes, reducing hospital admissions, preventive medicine, and patient experience.

POSITION OVERVIEW

• Value-Based Care model
• Medicare patient population
• 13-16 patients per day
• 20-minute regular visits
• 40-minute full diagnostic and new patient visits
• Care team includes a Scribe, Medical Assistant, and Social Worker
• On-site labs with a phlebotomist
• Access to RubiconMD for specialist access
• Average of 3-5 providers per clinic, including NPs and Primary Care Physicians

COMPENSATION

• $230,000-$250,000 base salary
• Bonus based on quality metrics
• Sign-on flexibility for Primary Care Physicians
• Relocation package considered on a case-by-case basis

• Monday-Friday
• 8:00am-5:00pm

ROLE

• Provide value-based healthcare to Medicare patient populations
• Focus on quality outcomes and preventive medicine
• Work within a care model designed to reduce hospital admissions
• Work with a care team including a Scribe, Medical Assistant, and Social Worker
• Utilize on-site lab services and access to RubiconMD
• EMR systems: Canopy and Greenway

Requirements

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate REQUIRED
• Internal Medicine or Family Medicine Board Certified or Board Eligible REQUIRED
• Active, non-probationary, unrestricted state license REQUIRED
• Spanish speaking REQUIRED
• No residents REQUIRED

Preferred

• Fellowship training in Geriatrics
