Bilingual Nurse Practitioner - Primary Care (Spanish)

Oak Street Health seeks a Nurse Practitioner to deliver effective value-based primary care to adults enrolled in Medicare. You will work within a collaborative care team in outpatient practices, conducting visits, health assessments, and care coordination to keep patients healthy and out of the hospital.

Responsibilities include annual wellness visits, patient education, and partnering with physicians and specialists to provide comprehensive care.
