To empower members in managing their diabetes, the full-time remote Bilingual Diabetes Educator (CDCES) will guide a panel of members through their healthcare journey, focusing on education and counseling via virtual communication to promote self-management and stability. Key responsibilities Screen members for eligibility and conduct detailed assessments for ongoing diabetes management education Collaborate with healthcare providers to develop care plans and optimize medication regimens for members Utilize motivational interviewing techniques to influence positive behavioral changes and achieve collaborative SMART goals Required qualifications Active unrestricted Compact Registered Nurse license or Registered Dietitian license Active Certified Diabetes Care and Education Specialist (CDCES) certification Minimum of 2 years of experience in clinically managing diabetes Ability to obtain additional licenses in states where required Experience with diabetes technology, such as CGMs and insulin pumps, is preferred
