Bilingual Clinical Care LVN

To support a growing Remote Patient Monitoring Department, the full-time Bilingual Clinical Care LVN will conduct patient outreach through calls, monitor health alerts, and collaborate with healthcare providers while working remotely in a contact center environment. Key Responsibilities Conduct outbound and inbound calls to check on patients and address health concerns, managing a call volume of 70 to 90 calls per day Monitor and respond to Remote Patient Monitoring alerts, escalating concerns as necessary Document all patient interactions in real time and perform monthly wellness assessments Required Qualifications Active LVN/LPN license required Bilingual proficiency in English and Spanish Proficient with computers, EMRs, and telehealth tools Strong communication and organizational skills Preferred: At least 1 year of nursing experience, particularly in RPM, telehealth, or chronic care
