Bilingual Case Management Nurse (RN)

To support comprehensive patient-centered care, the full-time Bilingual Case Management Nurse (RN) will assess patient needs, develop individualized care plans, and advocate for patients while working remotely. Key responsibilities: Coordinate and manage healthcare services to ensure effective communication among providers Assess patients' medical needs and develop individualized care plans aligned with their health goals Monitor patient progress and adherence to care plans, adjusting interventions as necessary Required qualifications: Active, unrestricted Registered Nurse (RN) license in state of residence; Compact License (NLC) accepted 3+ years of nursing experience in an acute care setting Fluency in both English and Spanish, verbally and in writing Demonstrated critical thinking and sound judgment in complex situations Excellent communication and interpersonal skills with a collaborative approach
