Bi-Lingual RN Care Team

To enhance patient care, the remote Bi-Lingual RN Care Team will manage patient assessments, coordinate care plans, and provide education to clients across various locations. Key responsibilities: Conduct patient assessments and develop individualized care plans Coordinate with healthcare providers and community resources to ensure comprehensive care Educate patients and families on health management and available services Required qualifications: Active RN license with bilingual proficiency in English and another language Experience in care coordination or case management Strong understanding of healthcare systems and community resources Ability to work independently in a remote setting Compassionate approach to patient care and advocacy
