Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adult Medicare patients in underserved communities. In this role, you will deliver bilingual (English/Spanish) counseling, conduct psychosocial assessments, and develop evidence-based treatment plans. You will collaborate closely with primary care providers, care managers, and community resources to support whole-person, preventative care.
Using your LCSW, LCPC, or LMFT training, you will offer brief interventions, crisis support, care coordination, and patient education, while contributing to a patient-centered, inclusive, and outcomes-driven care model.
