Qualifications & Skills:
- RN license with a Nursing Diploma/Associate’s
- 2-5 years of experience
- Preferred: Bachelor’s Degree in Nursing
- Strong communication, critical thinking, and cultural competency skills
Key Duties:
- Assess patient preferences, cultural, emotional, and spiritual needs
- Develop and document individualized care plans with measurable goals
- Collaborate with team members, patients, and caregivers to ensure continuity of care
- Evaluate patient progress, communicate changes, and update care plans accordingly
