RiverRidge Outpatient - ME seeks a Registered Nurse to provide compassionate outpatient behavioral health care to adults, adolescents, and families. In this patient-centered, community-focused setting, you will complete nursing and behavioral health assessments, develop care plans, administer medications, and support crisis intervention. You will collaborate closely with therapists, psychiatrists, and other team members, maintain accurate EHR documentation, and provide education to clients and families to promote emotional wellness and continuity of care.
Key Responsibilities
- Perform comprehensive nursing and behavioral health assessments for outpatient clients.
- Develop, implement, and evaluate individualized care plans.
- Administer medications and monitor for effectiveness and side effects.
- Coordinate care with therapists, psychiatrists, and other multidisciplinary team members.
- Provide patient and family education on mental health conditions, treatments, and self-management.
- Document all care, assessments, and interventions in the EHR accurately and timely.
- Support crisis intervention, de-escalation, and safety planning as needed.
- Participate in team meetings, case reviews, and quality improvement initiatives.
