Job Summary: In accordance with FLACRA's mission, vision, and values, the Behavioral Health Community Services Coordinator is responsible for coordinating community-based behavioral health services, including referral management, intake coordination, service assignment, data tracking, reporting, and communication with referral sources. The Coordinator also maintains a caseload and provides rehabilitative and recovery-oriented services in accordance with New York State requirements, managed care guidelines, and agency policies.
Services may include those authorized through Health Home, HARP, HCBS, CORE, or other community-based behavioral health programs as applicable.

Duties
- Coordinate referrals into community-based behavioral health services, ensuring timely communication with referral sources, care managers, and internal staff.
- Monitor referral status, authorizations, service thresholds, and client engagement to promote timely access to services.
- Assign referrals and coordinate intake activities in collaboration with program leadership.
- Maintain program data, tracking systems, reports, and performance metrics as required by the agency and funding sources.
- Maintain an assigned caseload and provide community-based behavioral health services in accordance with approved service authorizations.
- Complete comprehensive assessments, service plans, crisis plans, care plan updates, discharge planning, and other required documentation.
- Collaborate with Health Home Care Managers, managed care organizations, treatment providers, and community partners to coordinate client care.
- Participate as an active member of interdisciplinary treatment teams, including treatment planning and case conferences.
- Provide rehabilitation, recovery, skill-building, advocacy, and supportive services that promote client independence and recovery.
- Conduct community-based visits, including home visits and other approved community locations, as clinically appropriate.
- Assist clients in accessing medical, behavioral health, vocational, educational, housing, and other community resources.
- Provide transportation or transportation coordination when appropriate and consistent with agency policy.
- Respond appropriately to crisis situations and follow agency protocols to ensure client safety.
- Maintain compliance with all documentation standards, Medicaid requirements, managed care requirements, agency policies, and applicable federal and state regulations.
- Participate in staff meetings, supervision, required trainings, and quality improvement activities.
- Assist with onboarding and training new staff assigned to community-based behavioral health services, as requested.
- Maintain current knowledge of New York State behavioral health regulations, HCBS, CORE, HARP, Medicaid, and other applicable program requirements.
