ASVH-Flagstaff – LPN/RN Wound Care Nurse
Location: 2100 N Gemini Rd, Flagstaff, AZ
Schedule: Flexible (7am–3pm, 8am–4pm, or 8am–5pm), Monday–Friday, 8 hours/day. No weekends.
Position: 1 Full-Time
We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide specialized wound care and support residents’ skin health.
Key Responsibilities
- Perform wound assessments, management, and reassessments per policies and procedures.
- Develop and implement skin care programs to prevent breakdown.
- Manage residents with draining wounds, fistulae, ostomies, and incontinence.
- Revise and evaluate plans of care; assess and manage pain.
- Serve as a resource for nursing staff and provide in‑services on wound care, ostomy care, and prevention strategies.
- Communicate effectively with staff, physicians, and interdisciplinary teams.
- Maintain accurate documentation and stay current on wound care advancements through training and continuing education.
Requirements
- Current Arizona RN or LPN license.
- Certification as a Wound, Ostomy, and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN).
- CPR/BLS certification.
- Fingerprint Clearance Card issued by the Arizona DPS.
- In‑state travel as needed.
Preferred
- Experience in a skilled nursing or long‑term care facility.
