MAIN FUNCTION:

Responsible for assessing patients to determine appropriate level of care.
n n - n
Responsible for completion of assessment and admission documentation.
n n - n
Responsible for making referrals as necessary.
n n - n
Understanding of Addiction, Medication for Addiction Treatment (MAT), and the stigmatism associated with terminology and treatment of addiction and MAT.
n n - n

ESSENTIAL DUTIES AND RESPONSIBILITIES:
n - Ensure that all clients are treated with dignity and respect.nnnn - Affirming care for Lesbian, Gay, Bisexual, Transgender and Questioning clients.nn- n
- Perform all aspects of patient intake process including but not limited to admission- assessment, communicable disease risk assessment, CIWA Scale, COWS Scale Mental Status Exam, and toxicology testing. n
-Collect toxicology samples (urine or saliva) from patients, ensuring proper labeling, handling and adherence to Universal Infection Control Precautions.

Completion of all OASAS required admission documentation including LOCADTR and consent forms.
n n - n
Determine appropriate level of care, after reviewing case with supervisor.
n n - n
Coordinate all inpatient referrals with Intake and Referral and the Utilization Review Department.
n n - n
Communicate with other Departments (i.e. Medical, Fiscal, Intake & Referral, Clinical, Administration, etc.) to relay important clinical information.
n n - n
Provide individual, group and family counseling as needed.
n n - n
Enter Progress Notes and other Clinical documentation into EMR.
n n - n
Participate in staff meetings.
n n - n
Facilitate the completion of all necessary consent for release of information forms
n n - n
Act as liaison with community agencies to facilitate referrals
n n - n
Participate in weekly Case Conference meeting for treatment review of patients.
n n - n
Attend weekly clinical supervision.
n n - n
Ensure that Seafield’s policies and procedures and federal regulations regarding confidentiality of the patient and their records are adhered to.

Perform other duties as assigned.

EDUCATION AND QUALIFICATIONS:
nCASAC or CASAC Eligible, or B/A-B/S with working knowledge of Substance Use Disorders. Working knowledge of Electronic Medical Records and Managed Care.
