PRN Assessment Coordinator
Palmetto Lowcountry Behavioral Health is now seeking to fill a PRN Assessment Coordinator position. The Intake Counselor serves as the first point of contact for referral sources and potential patients to schedule assessments and/or admissions. The Intake Counselor thoroughly assesses and completes the assessment tool for all clients, communicates information with the on-call provider and nursing to determine an appropriate level of care, documents all findings and recommendations, assists clients in being admitted or referred out to the service or resource to best address their needs, manages referrals and ensures prompt response times, provides information to individuals, agencies, facilities, and professional staff requesting information regarding potential admissions, completes insurance verification and precertification as applicable, and participates in crisis and safety interventions as needed.
Qualifications include a Master's degree in Social Work, Psychology, Counseling or other related field, or RN, with at least one year experience utilizing assessment skills preferred. Age-specific competencies include Adolescent, Adult, Geriatric. Licensure/certification required includes CPR and CPI. The position has access to medical records for quality compliance and risk related issues, with use of PHI for purposes of healthcare operations.
