Seeking an Arizona Licensed High Risk RN, the full-time Remote Care Manager will coordinate care for Senior Health patients by developing action plans, managing transitions of care, and collaborating with care team members across multiple states. Key responsibilities Participate in daily huddles and weekly rounds to review patient care needs and triage calls Create patient-centered action plans and manage care transitions, ensuring effective communication among healthcare teams Address post-discharge barriers and facilitate access to necessary resources for patients Required qualifications AZ Licensed Registered Nurse (RN) with the ability to obtain licensure in additional states 5+ years of RN experience, including at least 1 year in care coordination or case management Experience in complex care settings, particularly with senior health and home-based care services Advanced knowledge of care management principles and chronic disease education Strong interpersonal communication skills and proficiency in using technology for patient care
