We are seeking a compassionate and skilled licensed physical therapist to join our team in a vibrant regional healthcare setting in the Midwest. The primary responsibility is to evaluate, treat, and monitor patients while providing ongoing assessments to support their rehabilitation journey.

Key Responsibilities:

• Conduct thorough patient evaluations and develop individualized treatment plans using approved examination procedures.
• Communicate treatment plans effectively to patients, families, caregivers, and the care team.
• Maintain accurate and timely documentation including evaluations, progress notes, discharge summaries, and logs in compliance with federal and state regulations.
• Collaborate with rehabilitation team members to coordinate comprehensive care and select appropriate treatment programs.
• Create effective home exercise programs to enhance patient outcomes in their living environments.
• Develop and implement evidence-based treatment protocols tailored to patient-specific goals.
• Respond promptly to referrals and build strong rapport with referring healthcare professionals.
• Supervise physical therapy assistants and support staff, ensuring adherence to regulatory standards.
• Assist with equipment maintenance and manage supply inventory.
• Participate in departmental management and present educational in-services as needed.

Education and Certification:

• Bachelor's, Master's, or Doctorate degree in Physical Therapy (BA, BS, MA, MS, or DPT).
• Current physical therapy licensure and board certification as required by state regulations.
• Up-to-date Basic Life Support (BLS) certification from the American Heart Association.
• Completion of Mandatory Reporter training within six months of hire.

• Demonstrated ability to develop and modify treatment plans based on patient progress.
• Strong communication skills and a positive, service-oriented approach with patients, families, and colleagues.
