CAN Community Health seeks an APRN / Non-Physician Provider to deliver compassionate, patient-centered care to individuals affected by HIV, hepatitis C, STIs, and related conditions. In this mission-driven role, you will perform advanced clinical assessments, diagnose and manage acute and chronic conditions, prescribe and monitor antiretroviral and related therapies, and develop individualized care plans. You will collaborate closely with an interdisciplinary team, provide culturally competent education and counseling, support prevention and outreach initiatives, and document care in the EHR.
