Job Description: We are seeking a compassionate and experienced Family Medicine Nurse Practitioner (FNP) to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Pepperell, MA. Qualifications:

• Master’s degree in nursing 2+ years of Family Medicine experience Strong clinical skills, including assessment, diagnosis, and treatment planning. Excellent communication, collaboration, and interpersonal skills. Proficiency with EPIC EMR preferred Ability to work independently and as part of a team.

Work Shift:

M-F 8am-5pm - position will be in MA
